Bendrofluazide is a thiazide diuretic — commonly called a water tablet — that helps the kidneys remove excess salt and water from the body through urine. It has been used for decades to treat high blood pressure and fluid retention (oedema), and remains one of the most cost-effective and reliable options for long-term blood pressure management. Available in two strengths, the dose is tailored to the condition being treated.

How does bendrofluazide work?

Bendrofluazide acts on the kidneys, blocking the reabsorption of sodium and chloride (salt) back into the bloodstream. When excess salt is excreted in urine, water follows — reducing the total volume of fluid in your blood vessels. Less fluid means less pressure on artery walls, which lowers blood pressure. In oedema, this mechanism relieves the uncomfortable build-up of fluid in tissues such as the ankles, legs, and lungs.

What is bendrofluazide used for?

High blood pressure (hypertension) — 2.5mg

One of the original first-line medicines for hypertension. At 2.5mg, it lowers blood pressure effectively over 24 hours with once-daily dosing and is often used alongside other blood pressure medicines such as ACE inhibitors, ARBs, or calcium channel blockers.

Fluid retention / oedema — 5mg

Relieves swelling caused by fluid accumulating in the body's tissues — commonly seen in heart failure, liver disease, kidney conditions, and certain medications. The 5mg dose provides a stronger diuretic effect for managing oedema.

Heart failure (as part of combination therapy)

Used alongside other heart failure medicines to reduce fluid overload, relieve breathlessness, and decrease the burden on the heart. Always prescribed as part of a broader treatment plan.

Do not stop taking bendrofluazide suddenly without your doctor's advice — blood pressure may rise and fluid can accumulate rapidly if treatment is discontinued abruptly.

Who should not take bendrofluazide?

✕Anyone allergic to bendrofluazide, sulfonamide-derived medicines, or any ingredient in the tablet
✕Patients with severe kidney failure (anuria) or severely impaired kidney function
✕Patients with untreated low potassium (hypokalaemia) or low sodium (hyponatraemia)
✕Patients with Addison's disease (adrenal gland insufficiency)
✕Patients with refractory hypercalcaemia (persistently high blood calcium)
✕Pregnant or breastfeeding women — use only if clearly necessary under close medical supervision

✓Stay well hydrated, especially in hot weather or during exercise — the diuretic effect increases the risk of dehydration.
✓Store below 25°C in a dry place, away from direct sunlight. Keep out of reach of children.

Low potassium (muscle cramps, weakness)
Dizziness or lightheadedness
Mild nausea or stomach upset
Raised blood glucose or uric acid
Skin sensitivity to sunlight (photosensitivity)

Most side effects are mild and often settle as your body adjusts. Contact your doctor promptly if you experience severe muscle cramps, significant weakness, irregular heartbeat, or signs of gout — these may indicate low potassium or raised uric acid levels requiring attention.
